Group Health Benefit Plans

Individual and Families Under 65 Health Plans
With BCBSF, individual health insurance has never been so affordable. We have many options customized to your needs, including comprehensive coverage and limited benefits plans. Plus, many plans have optional maternity benefits and Child Only coverage for dependent children ages one to 17.

- Miami-Dade Blue – in partnership with Miami-Dade County, local network plans offer valuable and very affordable benefits for the uninsured working or living in Miami-Dade County.
- Advantage65 – includes many network-based, Medicare plans that provide hospital and/or medical coverage along with your existing plan.
- CompCoverage – offers several plans that allow you to choose your own providers, in- or out-of-network and covers your Medicare Part A and Part B copayments.
- BlueMedicare HMO (offered by Health Options, Inc.) – features complete Medicare coverage including prescription drug benefits and dental services by network providers.
- BlueMedicare PPO – features prescription drug coverage and dental benefits, plus the freedom to choose any provider, in-network or out.
- BlueMedicare PFFS (Private Fee for Service) – offers flexible, comprehensive benefits for services by any Medicare-approved provider, and includes prescription drug coverage.
- BlueMedicare Rx – offers low cost Medicare Part D prescription drug plans, some with zero deductibles, available even if you do not carry BCBSF insurance.

FamilyBlue is not insurance. The plan provides discounts for services received from certain health care providers. The plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all health care services, but will receive a discount from the health care providers who have contracted with the discount medical plan organization. The range of discounts for services offered will very depending on the type of provider and the service. This plan is administered by Coverdell & Company, Inc., a discount medical plan organization located at 8420 W Bryn Mawr, Suite 700, Chicago, IL 60631, 1-800-918-7600.

BlueMedicare HMO, a federally qualified HMO, BlueMedicare PPO and BlueMedicare PFFS have a Medicare Advantage Contract approved by the Centers for Medicare & Medicaid Services (CMS). Blue Cross and Blue Shield of Florida, Inc. (BCBSF) contracts with the federal government to administer BlueMedicare Rx, a Medicare Part D Prescription Drug benefit in the state of Florida. These contracts are renewed annually, and availability beyond the end of the current contract year is not guaranteed.
A Medicare Advantage Private Fee-for-Service Plan works differently than a Medicare Supplement Plan. Your doctor or hospital is not required to agree to accept the plan’s terms and conditions, and thus may choose not to treat you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may choose not to provide health care services to you, except for emergencies. Dental, Disability and Life plans are issued through Florida Combined Life Insurance Company, Inc. (FCL), a BCBSF affiliate.
The Health Reimbursement Account (HRA) program and the Flexible Spending Account (FSA) program are not insurance programs, but a financial reimbursement plans. BCBSF, through its subsidiaries, Florida Combined Insurance Agency and Florida Combined Life Insurance Company, Inc., provide administrative services only for these programs and are not liable for any account balances. These programs reimburse qualified expenses as defined in the employer’s plan documents. The employer’s HRA plan document or the FSA plan document terms prevail over any inconsistencies in any verbal, written, or electronic information provided by BCBSF.
